As a Hospice Registered Nurse for General In-Patient Care (GIP), your mission will be to deliver compassionate and personalized patient care, in alignment with physician orders, to meet the needs of individuals and families while admitted at the hospital.   The GIP RN admits, plans, organizes and directs hospice care for General Inpatient (GIP) clients at our contracted hospitals and is experienced in nursing, with emphasis on community health education/experience. 

Responsibilities include:
  • Regularly assesses and reassesses the nursing needs of the Hospice patient. Competently implements care needed for comfort and palliation of symptoms.
  • Provides direct client care and works closely with case manager in implementing care.
  • Performs admission procedure as per RVNAHealth policies using electronic health record in a timely manner with accurate documentation.  Acts as liaison for admission to hospice, obtaining needed documentation for medical records, and acting as contact for hospital staff. 
  • Develops a plan of care consistent with the physician’s plan of treatment and patient/ family wishes.
  • Provides Hospice nursing services, treatments and preventive procedures.
  • Initiates nursing procedures appropriate for the patient’s Hospice care and safety.
  • Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient’s physical or emotional condition.
  • Coordinates the implementation of the plan of care for patients in the hospital setting.
  • Provides dietary counseling.
  • Teaches, supervises and counsels the Hospice patient, family members and facility staff about providing care for the client.
Experience:
  • Graduation from an accredited school of nursing; BSN preferred
  • Currently licensed to practice as a Registered Nurse in the State of Connecticut.
  • A minimum of two years of nursing experience, preferably in medical/surgical, oncology , Critical Care, ED or labor and delivery nursing. Home Health Care and/or Hospice experience preferred. Active patient contact within past three years, preferred.
  • Ability to work independently, make accurate, and at times, quick judgements.
  • Able to drive in all types of weather.

Compensation: $98,500.00 - $106,500.00 per year
